Case Manager - Homeless Youth Programs - Full time (Plainfield VT)

The Homeless Youth Programs Case Manager provides engagement and case management services to youth ages 18-24 who are living in Elevate Youth Service’s HOP House program. Caseload will include approximately 8-10 youth. Responsibilities include intake and assessment; individual and group counseling; crisis intervention; case planning; resource brokerage and referral; case coordination and advocacy; follow-up and after care; timely provision of clinical and financial record keeping related to job duties. Additional case management may be provided for other youth aged 16 through 24 who are homeless or at risk of becoming homeless but are not immediately housed by EYS. Youth Engagement Specialists - Overnight and Subs (Barre VT)

Elevate Youth Services is seeking Youth Engagement Specialist substitutes for all shifts, including nights and weekends and Overnight staff, for Return House - a residential housing program based in Barre, VT for young men under the care of the Department for Children and Families. Key to this program is the positive relationships our staff develop with residents to help them transition back to their communities. Youth served by the program are between the ages of 16 through 22 and have a history of DCF involvement that could include homelessness, involvement with the Juvenile Justice system, and behavioral support needs. Return House is a residential facility that provides youth with relationship-based conflict management as an alternative to the use of physical restraints. Learn more about this opportunity.
